Biography

Chelsea Cabrera is a registered nurse and dedicated care coordinator with UCF Health, proudly serving patients in the Central Florida community she has called home her entire life. Born and raised in Oviedo, FL, Chelsea brings a rare combination of deep local roots and broad clinical experience to her role — giving her patients the benefit of a provider who truly understands this community and is personally invested in its health and well-being.

Background

Before joining UCF Health, Chelsea built a rich and varied nursing career spanning long-term care, dementia care, medical-surgical nursing, and hospice and palliative care. The majority of her professional experience has been devoted to elderly care and end-of-life medicine — disciplines that demand exceptional empathy, clinical skill, and the ability to guide both patients and families through complex health journeys. This background continues to shape her patient-first approach every day.

Chelsea’s path to nursing is one of service. Prior to entering healthcare, she served four years in the U.S. Navy aboard the USS Ronald Reagan, an experience that instilled in her the values of commitment, discipline, and mission-focused teamwork. Those qualities are evident in everything she brings to her patients at UCF Health.

Chronic Care Management

Chelsea is a proud member of UCF Health’s CareConnect Chronic Care Management Program — a specialized service connecting patients who live with chronic conditions to the continuous, coordinated support they need. Through the CareConnect; program, Chelsea partners closely with patients to develop personalized care plans, support medication management, track progress toward health goals, and connect individuals with the community resources that help them thrive between office visits.

For Chelsea, chronic care management is more than a clinical function — it is an opportunity to build lasting, trust-based relationships with the patients she serves. She looks forward to getting to know every patient as an individual, understanding their unique goals and circumstances, and walking alongside them on the path to better health.
