Cardiology

Areas of Interest: Echocardiography, Cardiology Consultation

Requested Documentation: Please send in advance: Prior cardiac records (last 5 years), lipid profile (last 12 months), Diagnostic reports (echocardiograms, catheterization reports, stress test, Holter monitor, EKG) and any other imaging reports (CT/MRI).

Dermatology

Areas of Interest: 

Medical Dermatology

  1. General Dermatology (psoriasis, eczema, acne, warts, ring worm, nail fungus, hidradenitis supprativa,  moles, skin cancer screening)
  2. Specialty Focus- autoimmune and autoinflammatory diseases (pemphigus and pemphigoid, lupus, dermatomyositis, vasculitis, sarcoidosis, and lichen planus)

Procedural Dermatology

  1. Skin cancer treatment- Mohs micrographic surgery and excisions
  2. Cosmetics and other procedures (Botox, flillers, laser treatments)

Thyroid Nodules

  1. Thyroid ultrasound
  2. Biopsy report  (if applicable)

Hypo/Hyperthyroid (Graves disease)

Diabetes

  1. Hemoglobin A1c (within last 3 months)
  2. LDL (within 1 year)
  3. Creatinine (within 1 year)
  4. Urine microalbumin/ creatinine ratio (within 1 year)
  5. Ophthalmology Consultation Report (within 1 year)

Adrenal or Pituitary Conditions

  1. Prior labs used in diagnosis of adrenal or pituitary condition
  2. Last adrenal imaging study or last MRI of the pituitary (if applicable)

Calcium Disorders

  1. Hyper/hypocalcemia: Calcium levels, PTH level
  2. Hyperparathyroidism: Calcium level, PTH level, vitamin D level

  • Patients with abnormal labs such as very high ANA, RF, CK, Aldolase
  • Rheumatoid Arthritis
  • Ankylosing Spondylitis, Psoriatic Arthritis, Gout
  • Lupus
  • Sjogren’s Syndrome, Scleroderma, Mixed Connective Tissue Disease
  • Dermatomyositis and Polymyositis
  • Polymyalgia Rheumatica
  • Osteoporosis and Osteopenia
  • Vasculitis (many types: Wegeners, Churg-Strauss- EGPA/GPA,)
  • Sarcoidosis
  • Recurrent Uveitis that needs systemic medications

  • Referral with diagnosis and demographics attached
  • Recent Labs (including but not limited to: ANA, RA Factor, CRP, & Sed Rate)
  • Last office note from referring provider
